1. Acknowledgment of Risk
I, the undersigned parent or legal guardian, understand that participation in martial arts and physical activities at Mitten Muay Thai Gym (“MMT Gym”) involves inherent risks of injury to my child, including but not limited to bruises, sprains, fractures, concussions, or other physical or emotional harm. I voluntarily allow my child to participate with full knowledge and acceptance of these risks.
2. Assumption of Responsibility
I certify that my child is physically fit to participate in MMT Gym’s kids program and has no medical condition that would prevent safe participation. I agree to communicate any relevant health concerns to MMT Gym staff before my child engages in any activity.
3. Release of Liability
In consideration of my child’s participation, I hereby release, waive, and discharge MMT Gym, its owners, instructors, employees, agents, and affiliates from any and all liability, claims, or demands arising from injury, illness, accident, or loss sustained by my child while on the premises or participating in any program-related activity.
4. Indemnification
I agree to indemnify and hold harmless MMT Gym from any claims or damages resulting from my child’s participation, including reasonable attorney fees.
5. Medical Treatment Authorization
In the event of a medical emergency, I authorize MMT Gym staff to seek appropriate medical care for my child. I understand that I am financially responsible for any such care.
7. Legal Agreement
This agreement is governed by the laws of the State of Michigan. If any provision is found invalid, the remainder shall still apply.
